Arizona ESA families need to provide curriculum for all supplemental purchases. Use ESA AZ CURRICULUM Forms as a template and tie purchases into simple to use fillable forms (math, science, social studies, English language arts, physical education). 

      For example:  The ESA AZ CURRICULUM math form may be submitted for reimbursement for the purchase of math books, pencil, paper, manipulatives for counting, calculator, Legos (teach fractions), and number charts.  Anything related to math can be submitted for reimbursement together on one form.
